looking for some questions that you guys ask or if you're an assistant, that you are asked about on a end of the year survey or evaluation. i like to get the other coaches thoughts on the year a week or so after seasons over on what they think we did good/bad need to change next year. also if someone has done this with their seniors i would like to see a copy of what you ask them. thanks,

Ask them the #1 thing they like best about the program and the #1 thing they would change (and why!)

FBDoc, Great question, however, may backfire if an HC really doesn't care about the opinions of the staff and will never change anything. Just a thought.

My HR Masters came in handy when I administered a self-evaluation to the players on the program. I asked question like
I felt respected by my coaches
I believe that we were a well disciplined team
I believe that study hall was a valuable use of time
and so on, and I used a 1-5 scale, 1 being Strongly Disagree- 5 being Strongly Agree. I then performed a gap analysis between where our coaches thought the players would select, and where they did. I noticed a lot of survey bias though (the kids being scared to put anything less than 5) so those kids that did all 5's I eliminated from the project, kids who did all 5's except 1-2 items, I dropped the 5's to 4's.
As far as an HC surveying his AC's that's an interesting side to it I never thought to look at.
I would ask the same as fbdoc said, the most important question is always what would you change and why. Another great question is to ask what would you like to see implemented and why. There's always q's like "was practice too long/short, was study hall useful (if you have it), should we have more time/less time for film, etc.
