Does anyone do "end of the season review" of the coaching staff by the players? This is my first year as a JV head coach and I would like to pass out a form to our players to get some feedback from them about their honest opinions on the season, and see what areas we could improve upon as a coach and coaching staff. I was thinking about asking them to review their position coach and also myself.
Does anyone else do this? What are some other things you would include on the form? I looked for an example in Templates & Forms, but I did not find anything. I am open to all suggestions and ideas.

I have heard of doing this in end of the year evalutation meetings, but mostly by seniors. The line of thinking is that the seniors will really be honest since they won't fear you holding their comments against them. Haven't heard of a JV team doing it, but I don't see why not.

I've done it with the seniors and/or underclassmen, and gotten some good comments. We think we're doing the correct thing, but the kids have some good ideas that we may have lost contact with. Underclassmen will, many times, tell you what they think you want to hear, even though you've told them you want the truth. I've made it anonymous and gotten a clearer picture of their true feelings. Of course, I've also been cussed out a few times.
