Does anyone have a template for underclassman exit interviews. I would love to see what other questions people are asking and format they are doing them in to be efficient but get some depth of discussion with their players.

At a previous school we had position coaches fill out a "player review" for everyone in their position group, so every kid had at least two coaches reviewing them (offense and defense). The HC then had end of season meetings with them to go over where they stood, what their goals were for the next season, the rest of high school, beyond high school, etc.
I believe the form we filled out for kids was a 1-10 scale on a variety of things - competitiveness, coachability, physicality, speed, agility, etc. Then also a general space for comments. I want to say it was 10 categories so the total of the 10 categories gave them a score on a 100 point scale, similar to a school score. We had different "goal" scores based on what year the kid was and what level of player they wanted to be.
