This is copied from our player self eval questionnaire. It should be a good starting point.
What did you find the most rewarding about last season? What did you find the toughest to deal with? In your mind, what will be a successful upcoming season? What personal goals do you have for next season? What team goals? What are you going to do this off season/summer to reach your goals? List any students that you would like to see out for football & why? (Seniors Only) - Next Year Being a senior, what leadership will you provide? Who else do you think will provide leadership? Are you interested in furthering your education beyond high school? If yes, are you interested in playing football?

smdh that is incredibly unhelpful. Look in the mirror. I don’t treat kids all the same. Everybody has different issues. We allow kids to speak about themselves. Some kids are sleeping in cars, other kids are distraught about boy or girl friend. Others are worried about there stuff being taken by siblings. Others don’t know where there next meal is coming from. In this area many times football is just a release. Academic are a symptom. The last thing they want or need is a questionnaire. Those conversations are all over the map.

And asking the questions planck posted, or similar styled questions designed to improve the kid's football program prevent you from discussing the things you mention here how?

Most of our post season discussions has nothing to do with football maybe two kids where fb will be part of post discussion. And that is probably average. Hence the anything and everything answer. And you, like plank, do not help. If the op or anybody wanted more, all they had to was ask.

I preface mine with two points: 1) I have worked at programs that phrase questions in such a way that they elicit answers that justify the HC and the program itself are correct. If you want to improve and get honest feedback make sure you allow open dialogue and write questions in such a way.
2) I coach at a private Christian school, so some questions include faith based components.
That being written, here is a copy of my player post season questionaire; in addition I have included the one we send to our parents. I know thats a whole other can of worms, but think of it this way: it can placate parents to an extent if they think you are listening.
PLAYERS SURVEY
Please be as honest and forthright as possible, and remember this survey is anonymous. Does playing sports, and more specifically being a part of this team, make you more accountable in each of the following ways: Spiritually, Behaviorally, and Academically (please explain)?
What Biblical lessons or truths do you remember learning about the most during the season?
In what ways could we improve Biblical integration in this program?
What general life lessons will you take from this season that you feel will help you grow as a person?
Do you feel that the program supported you academically? How could we improve in this aspect?
Did you feel that you were physically safe while playing and during practice? If “No” was there a specific instance or reason?

canesfan has a good template. We ask them about their strengths and their weaknesses. How they rate themselves in different areas (weight room, practice, classroom, etc). Anything they’d want to recommend we do to make things better. Things that they like about how we practice. They have an individual and a team goal. We don’t let them do anything with numbers really in that regard. Then the coaches straight up tell them what we thought about them in the previous season and what they need to do to get better. Brutal honesty. I think they’re really helpful, the kids who are bought in have really benefited from them. Even with Covid this year, most of our kids have met their goals or became better at what we asked of them. I told our QB in his meeting that he received very few votes for team captain as a junior and he needed to step up as a leader, and he’s without a doubt our undisputed leader of our team this year.

I think it is about time for you to go and use your 5th screen name..

I just sent an anonymous survey to my returners with 3 questions. 1) What is one thing that I currently do that you's like me to continue to do? 2) What is one thing that I don't currently do enough that you think I should do more often? 3) What can I do to make you more successful?
Asking fewer questions is easier for people to answer and doesn't feel like you are grilling them. Giving them anonymity allows them to be more truthful than if you were doing it face to face.
I'm not looking to have a kid beat around the bush and be afraid of giving me the hard truths. I don't want sympathy I want results

We do interviews, which allow for longer conversations and can support more questions. But we definitely should do anonymous short surveys too to make sure we're getting the info we need.
