Hey coaches, need your input starting this Monday, our plan for this year is for the Head Coach and the position coach to meet with each player 1 on 1 for 5-10 minutes to discuss current status, goals, and expectations entering the 2014 season. I would love to get your opinion on the forms I have attached. My plan for the first page (evaluation form) is to have the player and the coach fill one out before hand then compare the results, then the player will fill out the goal sheet as we discuss them. The more we research this stuff, I am starting to think it is more important than any X and Os. That is pretty much everything we do for off-season evaluations. I met with all the linemen in from Frosh to Juniors in February. I think doing it then is good because you can refocus their minds on what they need to do (i.e. be in the weightroom) if they want to have a successful year.
The one thing we added this year, which I enjoyed, was a personality/life questionnaire. Favorite move, favorite music artist, worst moment in your life, best moment in your life, what do you want to be, who do you admire, if you could meet anyone alive or dead who would it be and why?
Sure it has nothing to do with football, but it helps build a rapport with the kid and sometimes you get answers that give a little insight on what is going on at home or in his head.

I do agree with you macdiddy. Having a personality questionaire does help build a rapprt with the students. Our players love it when you can remember a small detail about them for it ensures them that coach does care about them beyond what they can do on the feild.
with our Juniors and Sophomores we ask them what their plans are after high school. we do so to help get the ball rolling so they can start looking at colleges to apply for

I think you got some serious typos on your form. Take a look at your SMART acronym on the 2nd page. You put SMART Specific Measurable Acheiveable Measurable Time dated
SMAMT
Other than that it looks great.
