I've been involved with and on teams that do it different ways. Team elections, coaches choices only, combinations, Game day captains. Which way works best for you?

We watch our seniors and juniors all through our preseason camps, and during 2 a days. We figure the captains will begin to appear during those days (guys that work hard, and help others, push others etc) After 2 a days we select 4 (the coaches do). Has never failed to work. Had a captain this fall that was not a starter to start the season, but worked his tail off, pushed others, never complained, and helped our younger kids. He earned a starting spot in week 5. He was a terrific leader from day 1.

we make our kids fill out an application in the spring and they must give a 2-3 min speech to the team why they would make a good captain. Then we let the kids vote on it. I would say it has been 99% effective. We tell the captains that any violation over the summer they will lose their opportunity to be a captain.

A focus for our program this season knighter! This year my goal is to get quality players in this position. Last season our captains weren't that great - too much ME, ME, ME. I invested in Jeff Janssen's Team Captain's Leadership Manual right after the season concluded. It has great information on the variety of methods a coach can take to choose captains and also has a 10 week program that takes your captains through some great material on what it means to be a leader. - this program will be a requirement for anyone selected as a captain in our program. It will require the captains to meet with me once a week for an hour or so to go over the material in the book and discuss other program-related topics.
I used to let our returning players vote. After reading this book, I am going to give my returning players a survey that asks them 10 questions. For each question they are asked to list up to three players, for example, "who you trust most on the team". Their answers will be tallied and serve as nominations. We will then take the top 5 vote-getters and as a coaching staff select our 3 permanent captains.
We also have an honorary captain each week. We have 3 "permanent" captains, then each week those three captains along with the coaching staff selects someone who has worked extremely hard during last week's practices or had an outstanding game to serve as "honorary captain" for the next week. This person will lead stretching with the other captains and agilities each day at practice and go out for the coin toss on Friday night.

we send the scout of the week out on friday for coin flip. 1 captain always allows scout of the week to be a part of the flip (since you can only have 4 there)
we also do a leadership program for all seniors...good stuff for all of them even if they are not going to be a captain.

We do almost exactly the same thing. The potential candidates also have an interview with coaches and a representative from each class.
We also have a leadership program for seniors. covering 5 weeks in the spring, and 5 weeks in the summer.

Kids vote at the end of 2-a-days for 2 captains. OC and DC pick weekly captains to go with.

Kids vote during 2-a-days. Head coach counts the votes and makes sure the right kids were selected. Usually the kids vote for the right kids. Usually take the top 3. They can turn down the honor if they do not feel they are up the the challenge of being a captain.

i like when kids ask for it and ask what they need to do to get it...then i see that they follow thru...if they do, theyre a captain. i choose 4 based on qualities that i like in my kids. good people, coachability, passion and dedication.

The way we choose captains is the coaches pick 2. Each week, we evaluate who had a good week of practice, good game prior, and choose 2 more. Doing this..we let a lot of kids have the ability to be a captain as well as have 2 guys (1 offense, 1 defense) that are the captain every week. The last game of the season, we pick 4 seniors, and the remaining seniors walk out with them behind the 4. It has worked well for us last season, kids really worked their butts off in practice to be 'noticed'.

Started something this year:
The team voted on 6 captains in the spring. Those guys are in competition to show who can lead the team the best. The coaching staff will pick 3 of those 6 to be in season captains. We also always have a game captain captain to reward a great week of practice.

Player vote on two Captains, coaches pick the other two Captains for the up coming week at the end of the previous week, so they know that come Sunday film they are in charge of running the room. On the field after the get together call goes out, the Captains take over running stretches/warm-ups, and at the end of practice for sprints the Captains can sit out. We also run the Enduro Drill where Captains can op out of it as well, but have never had one ever not stay the course. When told that they could drop just because they were Captains, between gasps I get back "Captains don't quite, we set the example." Even after they are no longer the Big C they still stick with the drill at least until they get bounced by a coach.
