Does anybody here have their players complete a mid-season program evaluation? What problems have you encountered? What positives have you experienced? Does anyone have sample evaluations they would be willing to share?
I have no idea if I would ever use one but I have read four sources from coaches who use them and have felt very good about them.

We don't do a "formal" mid season review, although I feel the idea has merit. However, we try and use our leadership council of players throughout the year as a way to review facets of our team including morale, conditioning, performance (players and coaches), and needs of the team. The open communication that this program has fostered has been a huge benefit to our team.
We also do a "Senior Sharing" night during our final regular season practice. Each senior is given time to share his words of wisdom with his team-mates. It might be relating his greatest play or favorite memory. Some have talked about their progression from JV scrub to Varsity starter (or star!). Most will mention one or more of their coaches and the impact they have had on their life. There's always at least one very funny story that gets retold. In the end it is without exception, a great affirmation of the program, the team, and each individual player.

we keep tabs with the Team Captains, but generally we do the program evaluations at the end of the year.
Unfortunately, these are KIDS and think like kids. Therefore, when you WIN, the evaluation reads, "This is the best program ever. We have the best coaches" And when you lose, it's "Aw this program stinks, these coaches don't know squat. I should've played more....yada yada "
