Does anybody pass out a player survey/questionnaire at the end of the season?
Wondering some of the questions you may ask, or even if there is a generic survey that you may share...

I have known some programs that setup the questions to illicit answers that justify the coaches; basically self-fulfilling self-gratifying.I think you have to watch out for that, unless that is what you are looking for.
Here is a copy of the questions I use:
Please be as honest and forthright as possible, and remember this survey is anonymous.
Does playing sports, and more specifically being a part of this team, make you more accountable in each of the following ways: Spiritually, Behaviorally, and Academically (please explain)?
What Biblical lessons or truths do you remember learning about the most during the season?
In what ways could we improve Biblical integration in this program? What general life lessons will you take from this season that you feel will help you grow as a person?
Do you feel that the program supported you academically? How could we improve in this aspect?
Did you feel that you were physically safe while playing and during practice? If “No” was there a specific instance or reason?
What offensive techniques, movements, or plays do you feel you learned best?
What offensive techniques, movements, or plays did you struggle with learning the most?
What defensive techniques, movements, or plays do you feel you learned best?
What defensive techniques, movements, or plays did you struggle with learning the most?
What aspects of our athletic training (weights, plyometric, stretches, running) do you feel best helped you?
What aspects of our athletic training (weights, plyometric, stretches, running) was most difficult?
Will you participate in the offseason training (or would you if you did not have another sport)?
Do you feel that all the players on the team were treated fairly by the coaches (please explain)?
What were your favorite and least favorite parts of practices?
What were your favorite and least favorite parts of the season as a whole?
What three things should the program get rid of to be successful?
What three things must stay for the program to be successful?
What changes would you make to help this program to continually improve?
Do you plan on playing football again next year?

Ours asks the following questions and asks our players to rank their answers 1 to 5 with a 1 being low.
1. How much fun was this season for you? 2. How much did you improve at football this season? 3. As a whole, do you feel like the coaching staff is Transactional (focus solely on winning and meeting their personal needs) or Transformational (use their platform to teach the Xs and Os, but also teach the Ys of life. They help young people grow into responsible adults)? 4. What is your favorite memory from your time as a Tokay Football player? 5. How can we improve as a program for next season? 6. What would you change about practice or the practice schedule? Why? 7. What was the most important lesson you learned during your time as a Tokay Football player? 8. What is the first thing you would change about your experience in this program? 9. What is your goal for after high school?
We've gained some valuable information from these questions in the past.

Ours is: Strengths/weaknesses of the program Strengths/weaknesses for you as a player How can we address those weaknesses? What can take the program to the next level? What role will you personally take in doing that? What will your role on the team be next season? Are you comfortable with that role? What could the coaches do a better job of while teaching? What could coaches do a better job of when motivating? Do you have any grievances? What player do you think most highly of and why? Goals for the off-season.
Seniors is vastly different.

Kids see this as just another test they have to take. They're KIDS! 5 years ago they were children. Seriously...Are you going to take into consideration what a teenager's evaluation is? Pass this stuff out to your AC's and let them evaluate anything they want...they're professionals. Seek out professional evaluations. The kids are ready to go indoors and play basketball.

I do a Google form and it takes about 3-5 minutes for them to fill out. I get a little feedback and also let the boys tell me what they think of the program. Non of them seem to mind doing it and I tie it into turning in equipment. So as they are standing in line turning in equipment they fill it out.

I remember one year some parents got mad at the head coach over playing time and started a petition to get him fired, something like 40 seniors signed it
I pulled a couple aside that I were the more mature type (I thought) and asked why they had signed "oh uh...I dunno...just kinda...all the other guys had signed it so I felt like he must have done something really bad" "did you think he needs to be fired" "oh...oh no not really I just signed it" "okay so you have no personal reason to want him fired you just signed it because somebody said sign it?" ".....yeah I guess so"
teenagers are idiots and shouldn't be consulted about anything except angst, bieber lyrics and how to use an e-cigarrette

I feel like I have gotten a much better understanding of where complaints will come from and how we can address them before it gets to the point of mob mentality.
Petitions are an absolute waste of time on almost every level. If your boss thinks they hold water you are in serious trouble. Our former superintendent received a petition from teachers a few years back...within about 30 minutes he sent a district wide email that basically said that if the teachers thought a mindless petition was an effective change agent that negotiations would prove to be a major challenge. He was correct.
