Last season, for whatever reason the shoulder pad clips (T-hook thing to clip-on the strap) kept falling off quite often for a lot of players. We ended up going through the whole quantity of emergency clip. Players would not noticed until after practice was over. Any idea what causes this and how we can avoid it happening more frequently? I thought it was due to the old straps but even the brand new straps were coming off.

If they are coming off that frequently, then they are improperly installed. Even then they still come off. It is such a pain in the butt it is forcing me to fundraise harder to buy the pads that only have buckle straps!
One solution to the problem is to get them fit and then use athletic tape to lock them in place.

It's how you put them on, you don't just go through the slits once, you go through and back. I'll try to explain...there are 3 slits, call them A, B, and C with A being closest to the T hook. Insert strap through slit C from underneath, then take it down through A from above. Next take it through slit B from below, followed by slit C from above, pull any slack out of the strap and you're good to go. If you have it this way, you will need to tape it, but I've never had issues unless the strap is too small width wise for the T hook.

Have them set them, then take medical tape and wrap it around, both front and back.
Honestly, if you have belts on your shoulder pads, they are kind of redundant.

tape them up at both ends if they are not sewn in, that is as old as the hills

My re-conditioner sales the clips for 0.39 cents. I usually get 50 of them about $20 bucks before every season...

all the newer ones we get are the buckle kind
but for older ones just set each up on the elastic strap to the ideal spot... tape over it
