We do two things, First we have the Seniors take a few min each to talk to the team about there time playing football. We also have a skill # and a line # that we pass down to a player that we the coaches and the player who wore it last, feel that would represent our program best.

We run the ball mainly, and our kids have hit the 5man sled all their career. Therefore the last thing we do on the last practice is have the seniors go through the routine while everyone else is around cheering them on. Once that is done the underclassmen line up along the back of the endzone and shake each senior's hand as they go by. Coaches are at the end of the line and do the same (usually ends up with some tears from the Sr.'s and them walking in telling the underclassmen how fast it goes by).

We had the seniors all do a reminder (run the length of the field doing an up-down at every 10 yards) and then it was tradition that each senior was carried off the field by their teammates all the way to the locker room. Apparently this tradition was started by our current DL coach. He also started the tradition of placing a signed cleat in the ceiling tiles when we clean out the locker room after the season ends.

We line the seniors up on the field (4 per yard line with them on the hashes and the numbers). The underclassmen and coaches snake through the line and tell the seniors thank you and give them a hug or handshake.
Then, our seniors will all meet together and get a break one last time.

Old HC used to have a ceremony under the goalposts of the practice field. Each senior would bring in an item from the season/their career to burn. He'd make a small fire, and each senior would place his item on the fire and then address the team about whatever was on his mind at the end of it all. It definitely wasn't the safest thing, but the HC had a nice little speech about how he'd take the ashes and spread them on the practice field so the seniors would always be there with the team in the future years. Seems kinda weird in the abstract, but it was pretty cool to see each year.

If you are in the playoffs how do you approach this?

A former HC I worked under for a year had the seniors stay after practice and run "their last mile" around the track together while the underclassmen went home.
Oddly enough, it was common for seniors at that school to quit late in the season...

We do ours the last Wednesday of regular season. Put all the seniors on one end and I speak to each one individually and then they are carried off the field by a few underclassmen.
