We finished up our season on Friday and we are looking at doing exit interviews with all of our players. What questions do you ask? For our seniors we will have different questions than our returners. Our returners we will focus on what they plan on committing to over the next 9 months to make next year a success.
Seniors we will focus on what they liked and what we can do better as a staff that they noticed over previous 4 years.

We don't do exit interviews with our seniors. I give them a lengthy sheet or questions and then I meet with them individually later. The underclassman exit interviews are more of us coaches giving feedback from the past season and what we are expecting out of them the next season. The main things I ask the starters/main contributors is what they they think their strength and weaknesses are. For the younger/scout team I also ask that and ask them if they think they are ready to play on Fridays.

If you give them a sheet make sure your questions arent phrased to get the answers you want. I know a guy whose questions are phrased as such:
Are we strong enough?
Are we fast enough?
etc. Phrasing it like this will almost always get kids to write "yes". Instead ask them which is the biggest area of concern: strength, speed, etc. If you consistently get the same answer then that would be indicative of a problem.

I haven't done this in two years, but intend to do so this year. This is the form I used three years ago and plan on using again this offseason. I try to meet face to face with all the kids to verbally do this vs just handing them a sheet of paper.
1. How did you think the season went for the team? For you personally? 2. If you could change anything about our program what would you change? 3. What are your goals for the team for next season? Personally? What position do you want to play 4. Where we see you- Position- Weight Room- Things to work on-

What do you think you were not prepared for in the game? Why did you come to practice everyday? If you could come back here and coach in four years, would you? What coach that you played for would you try to emulate with your coaching style? Why? What do you think we could have done differently to win more games? What are some personal goals that you feel you achieved? What are some personal goals that you feel you did not achieve? What are your two best memories from your senior season? When someone asks you why you played high school football, what will your answer be?
