I have one I used early in my career but discontinued because feedback wasn't particularly helpful, just more paperwork.

My AD does it for all athletes at the end of the season. Not sure what exactly he asked on it, but the players told me one. Did you feel the coaches were organized and ready for practices.
I know Coach Fore has done this as well and he is very open to helping other coaches out.

I have one, basic stuff: what did you like/ not like. Grade each coach; lots of specifics about your pos coach (did he communicate well, did he know his craft, etc.) Questions about the team process- did we run enough, did we lift enough. Personal questions- do you drink, smoke, ets (same questions about the team- do we have a drinking problem). Then questions about what youd like to see changed

I cant see the need for it. All youre doing is giving whiney and crybaby a platform. Anyone with real courage has already spoken to you about the program and built that relationship up where it needs to be so that they have some input, a voice. That's what captain status is for.

We use Survey Monkey at the end of the season and send out a 10-question survey our kids answer anonymously. We also do an Exit Interview with all returning players.

Agreed, you have to take who is saying what with a grain of salt. Kids who were unhappy probably wouldn't even fill it out any way.

In all the programs I've ever been a part of, instead of giving out a survey at the end of the season, the head coach meets with every player on the team, including outgoing seniors. This allows the player to feel like he is being listened to, instead of giving him a paper that most kids won't really put a lot of thought into and treat like another piece of homework. It takes time, but the feedback is much better IMO.

I know Coach Fore has done this as well and he is very open to helping other coaches out. Thanks for the kind words. I've written about this, and why I feel there IS a need to do this exact thing on a yearly basis. The humble coaches will want to know what they are doing - good, bad or indifferent. I started doing this my first year as a head coach, on paper, and have since transferred to Survey Monkey, where it's very very easy. Here is my article on doing surveys, at the end is the basic survey I've used since 2003. This one is even MORE eye opening for coaches. A survey not about the program, but about YOU as a Coach!
