hello
I was wondering if any coaches out there do player questionnaires before or after their season? I have always done with during 2-a-days that is pretty standard (contact info, goals, and a couple get-to-know-you questions) but I am thinking about giving one at our next workout. Maybe its my love of psychology but I do like getting a gauge on a player so I can get to know them better and get a better understanding of what makes them tick. If anyone else does this, please share. Do you think this is a good idea?
Here is the questionnaire I plan on handing out:
Full Name:__________________________________________
Home Phone: ( )_________________
Cell Phone ( )_________________
Email Address: _____________________________________
Parents’ or Guardians’ Name(s)
What sports do you like to play? Basketball ___ Baseball ___ Track ___ Wrestling_____
Who is the most influential person in your life?
What is your favorite subject in school?
What are your hobbies?
What grade would you give yourself for last season:__________
What grade would you give yourself this off-season so far:________
What do you want to improve the most before our first game?
What is one goal you want to accomplish as a member of this team for this year?
Where do you see yourself in 5 years from now?
What do you think could prevent us from being successful?
What do you plan on doing to make your team successful?
What do you feel are 3 characteristics a good teammate should have?
How can I help you become a better football player, and person?
What do you consider your strengths?
What do you consider your weaknesses?
When your career is finished, what do you want to be remembered for?
Are you willing to pay the price for success?
Good idea? Suggestions? thanks
