Lots of great replies to this, thank you guys for all the thinking fodder!
Think the process will be Vote --> Veto --> Train 'em.

We voted weekly and it was a ballot vote so nobody knew who got the votes. Put your name in the top right hand corner so we know who voted and put 4 names on the sheet. Top 4 vote getters got it. We had a center who went to TCU on a full ride who was never a captain. Kids knew he wasn't a hard worker and he never got a single vote in 2 years.

We had great kids who understood what being a captain meant. I think if you lay out what your idea of a captain is it helps kids to determine those characteristics when they vote. It also helps that it's a secret ballot. They aren't afraid to vote honestly if it's private.

If kids have a great game of course they talk to them but often the reporters ask me who can talk about the upcoming game or a pre-season review. True story. A few years ago the local paper asked to do a 'getting to know you' article on one of our players. So we let our senior mike backer do it, and this was one of the quotes that came out of the story: Q: What's your favorite video game? A: Grand Theft Auto V. It's pretty much real-life stuff. That's the reason I love it. You can do whatever you want. You can pretty much go to strip clubs and make money from bank robberies.

You can't be a captain for us unless you've earned 75% of the off-season work points and 75% of the summer points (sport practices, weight training sessions and camps all count for points). We'll let the kids vote after that but we have veto power. We've only vetoed a captain choice once. The kid was a multiple sport athlete (football, basketball, baseball) and managed to get all of his points through sports practices. He didn't attend many weight room sessions or single camp. We were alright with this as he was still active in the off-season but the kid was an arrogant a--. To call him "uncoachable" would be an understatement... He wasn't a leader and he wasn't going to see the field his senior year. But, the vote was a popularity contest for this particular team and this little sh-t managed to get votes. We shot it down in a hurry.

I'm now at a school where the HC is changing how captains are selected. This year the coaches will make a selection, stand the player in front of the team and then ask if there is any reason why the player should not be a captain. The HC said he's hoping to avoid what has happened in years past - a popularity contest rather than selection of true leaders.

The only thing I would add is, no matter what system you use make sure the kid actually wants to be a captain.
I think sometimes its easy for us coaches to assume that everyone wants to be a captain, but that's not always the case.
As a player the title never mattered to me. I was never a vocal guy, just showed up and did what I was asked.
Senior HS season, the coaches named me a Captain. My buddy wanted it in a big way. He was a solid player, always there, helping others, vocal etc and I think would have done a good job. (Hell he even recruited me to come play football in the first place.)
When I was named Captain, he was hurt and it was awkward in our relationship. If I was asked about it by any coach before being just announced in front of the team I would have declined.
As a coach, I always ask people privately first if they want to be a captain for this very reason. While clearly most say yes, I have had one kid say no thanks. (He felt it would be too stressful for him.)
I really like the application idea to remove that issue entirely.
