Has anyone used contracts for players and their parents to sign giving a second chance to participate in the program?

We have used them as a last resort. Each is unique in that it spells out what will and will not be tolerated.

We implemented some in the off-season when we had some players steal tip money from a fundraiser we did.
The contracts stipulated they do 10 hours of community service, apologize to the team in person, and write our booster club an apology letter prior to participating in any other off-season activity.
Prior to participating in summer practice, they had to do 10 more hours of community service for a total of 20 hours.
Also, they went to the bottom of the Pride Points list we keep and had no ability to move up. Basically, that means they would get their equipment and uniform number last. Any behavioral incident at school added 5 more hours to their community service requirement. Any F grade on a progress report or report card would remove the player from off-season activity until they pulled the grade up to a C. Each player had to pay the money back to the booster club and were banned from working any other fundraisers.
Had this happened during the season, we would have removed them from the team. Since it was our off-season, we gave them these requirements to meet.
